Health Requirements
Pennsylvania School Law, Section 1402, mandates physical and dental examinations in certain grades. The state encourages parents to have the family physician or dentist do the examinations since these doctors are aware of the student's health status and history. Examinations by the family physician or dentist is at the parent's expense.
We suggest that you have your child visit your physician or dentist over the summer since the doctors' offices are normally less crowded at that time. Please have the doctor or dentist return the form to the school nurse if it isn't completed during your appointment.
If you prefer, the school district's physician or dentist will provide the appropriate exams during the school year. There is no charge for the school exam.
Please return the completed official physical, dental, or scoliosis form during the first week of school to your school nurse.

Immunization Requirements - Students will only be admitted with proof of completed immunizations.
- Four doses diphtheria/tetanus (DTP), one dose to be given on or after the fourth birthday
- Three doses of polio vaccine
- Two properly spaced doses of measles-containing vaccine, usually given as measles / mumps/ rubella (MMR)
- Three properly spaced doses of Hepatitis "B" vaccine kindergarten and Grade 7
- Varicella Immunity
- Disease or vaccine on/after 1st birthday-K or 1st Grade
- Disease or two doses of vaccine if age 13-Grade 7
